Bring your insurance card and legal identification card when you visit. It is easier to upload your info before your visit using our secure patient portal. It is important that we have correct insurance information for correct billing. It is your responsibility to provide correct information.

Billing Policy: Important read carefully

Financial Policy – Patient Responsibility

At Omni Medical Center, we are committed to providing high-quality care to all our patients. Your billing statement will reflect billing from iCareNow or Omni Health Management Group, all part of Omni Medical Center. Please read our financial policy carefully:

1. Insurance Coverage: While we make every effort to bill your insurance, ultimate financial responsibility lies with you, the patient. If your insurance denies coverage for any reason, including but not limited to out-of-network status or non-covered services, you will be responsible for the full payment of services rendered. For you convenience you can upload your correct insurance information using our secure patient portal.

2. Payment Responsibility: You are required to make payment for any services not covered by your insurance plan. This includes co-pays, deductibles, and any balance remaining after insurance processing.

3. Insurance Denial: In the event your insurance denies coverage for services provided, regardless of the reason, you will be responsible for the full payment of your bill.

4. Updated Information: To avoid billing errors, please provide us with your most current insurance information at every visit. Failure to do so may result in claim denials and transfer of full financial responsibility to you.

5. Out-of-Network Care: If you choose to receive care from Omni Medical Center while out-of-network with your insurance plan, you may be responsible for higher out-of-pocket costs or the full cost of services.

6. High Deductible Plans: If you have a high deductible insurance plan, please be aware that you are responsible for paying the full cost of services until you meet your deductible. After your deductible is met, your insurance will begin to pay according to your plan’s benefits. It is your responsibility to know your plan’s deductible amount and to track your progress towards meeting it.

7.Billing Terms and Late Payments: Payment for services is due upon receipt of your statement. Any balance remaining unpaid after 30 days from the statement date will be considered late and subject to a late payment fee. Additionally, interest will accrue on overdue balances at a rate of 20% per month. Omni Medical Center reserves the right to send accounts with past due balances to a collection agency if no response or payment arrangement is made. Please note that accounts sent to collections may be reported to credit bureaus, potentially affecting your credit score. To avoid these consequences, we urge you to pay your bill promptly or contact our billing department to discuss payment options if you are experiencing financial difficulties.

8. Confidentiality waiver: If an account is processed by a third-party collection agency or a “past due” status reported to a credit card bureau, a record of your visit to Omni Medical Center may be come a public record. We reserve the right to deny treatment to patients who do not comply with our financial policies.

By receiving care at Omni Medical Center, you acknowledge and agree to these terms. We appreciate your understanding and cooperation in managing the financial aspects of your healthcare.

 

Self-Pay Patients:

We accept cash or credit card payment for self paying patients. Full payment is due at the time of service. Self-pay rates are lower than the insurance charges, inquire about self pay at the front desk or email us. Always ask for receipt from the front desk for payments made Omni Medical Center(iCareNow).
